Objective: To report the experience of primary health care nurses in using the Video-Observed Treatment (VDOT) system as a strategy for tuberculosis treatment surveillance. Methods: This is an experience report with a descriptive and reflective approach, systematized in five stages: starting point, initial questions, recovery of the lived process, in-depth reflection, and concluding points. Results: The findings of this study may contribute to the goals of the Programa Brasil Saudável (Healthy Brazil Program) by strengthening primary care, promoting equity, and incorporating digital technologies into tuberculosis care. The mediation of VDOT by nurses shows potential to improve treatment adherence and enhance surveillance, aligning with the program’s guidelines on technological innovation, integrality, and reduction of inequalities. The leading role of nurses, through active listening and adaptation to local realities, highlights the need for policies ensuring infrastructure, digital inclusion, and professional training, which are essential to advance tuberculosis control. Conclusions: It is concluded that VDOT, when implemented with institutional support and nursing leadership, represents a feasible and humanized strategy for tuberculosis treatment surveillance in primary health care.
